You (“Customer”) assume all responsibility for using each product as it is intended.
I authorize VMC LLC (“Vitality Modification Co”) to perform work outlined in this form on the items listed above, as well as, agree to the accuracy of the described project. I hereby release and agree to indemnify and hold harmless VCM and its employees, officers, and agents from and against any and all liability, claims, demands, actions, damages, expenses, and costs, including attorney’s fees, court costs, and loss of judgements of any kind and nature which may result from services provided for me by VMC LLC.

Terms and Conditions Vitality Modification Company (VMC)
Deposits – Vitality Modification Company (VMC), requires a deposit when we receive your firearm(s) or item(s) to be finished. Deposits are usually 50% of the total bill on the initial invoice. You may pay deposits with Check, Money Order, Cash, Credit Card. Deposits will be applied to your invoice. If the customer decides to cancel the finishing services before any work is completed, the deposit will be returned minus the cost of any supplies that were purchased or used for the job and shipping costs. If work has been performed, you will be charged for the work that has been performed. A deposit will be due before any work is performed unless otherwise advised & written on invoice by a manager from Vitality Modification Company (VMC).
Payments - A full payment will be required before my firearm or accessories are returned. Deposits are applied to your final bill. If full payment is not received, my firearm(s) or item(s) will be held for collateral until full payment is satisfied. Find yourself in this situation. **Forfeited firearms will be taken to our FFL to be transferred into our name. Delinquent accounts past due for 30 days or more will incur a one-time $20.00 late fee and a reoccurring monthly 3% charge to the total bill every month thereafter. If payment is not received within two months we reserve the right to collect on the account and or seize the customer’s assets which will become property of Vitality Modification Company (VMC). By not paying the bill you are authorizing us to transfer your firearms into our name. You agree that You will forfeit your firearm and it will become property of as Vitality Modification Company (VMC) and may be sold. Your deposit will not be returned. We will also reserve the right to refuse any future business with you.
Limited Warranty – Vitality Modification Company (VMC) will always let you know the results that may be expected; but we are not responsible for any misunderstandings or misconceptions pertaining to any finishes applied to a client’s firearm(s) or item(s). Vitality Modification Company (VMC) will attempt to resolve any misunderstandings that may arise, but the client is still responsible to pay for any services/work that has already been performed. I understand that Vitality Modification Company (VMC) makes no warranty on the results of custom work, including but not limited to; suitability of finish for any particular application, safety or reliability issues that result from the customization of the firearm(s) or item(s), durability of finishes, look of artwork or camouflage patterns and function. Factory engraving on the firearm will take on the new requested color(s). You understand that the work performed is like individual works of art – they are hand-painted not mass produced – that the photos are for reference only and do not represent the exact piece you will receive (the color palettes will be the same with some tonal variations from the pictures and placement of colors will be different within the same color palettes). If the coating wears off we will re-coat it for the lifetime of your gun, as long as it is determined not to be caused by abusive treatment. It is a coating and will scratch off for scratches we can re-coat those areas for a nominal fee.
Terms and Conditions – By having work completed through Vitality Modification Company (VMC) you automatically acknowledge our terms and conditions regardless if this paperwork has a signature or not. It is the customer’s responsibility to read and understand our terms and conditions. I understand that if I ship my firearm(s) or item(s) Disassembled they will ship the same manner as Vitality Modification Company (VMC) received them.
I take full responsibility to ensure that my firearm complies with the established laws in my local or. As Vitality Modification Company (VMC) reserves the right to take pictures of any and all work performed to be posted on our web page. We will not disclose any personal information unless given to us with your permission. Unless you are a dealer and or your gun is being consigned or sold at a dealer’s store. At your request we will post the location.
I understand that certain procedures may be necessary to prepare my firearm for custom work. These procedures may and or will include any of the following: dis assembly of firearm(s) and or item(s), removal of old finish and oil, sandblasting and some sanding of parts to be coated, Parkerizing with a magnesium phosphate solution and masking of areas.
Sights, optics or other aiming equipment will have to be removed. As a result some may experience a loss of zero.
At Vitality Modification Company (VMC) we use due care when handling your firearm(s) or item(s). We go to extreme measures to care for, secure and handle your firearm(s) or item(s) in a professional matter during the refinishing process. But sometimes through no fault of our own problems may arise with your firearm. Due to damage, age, use and abuse, corrosion and/or factory defect, some parts may fail during the dis assembly/assembly process.
I agree not to hold Vitality Modification Company (VMC) responsible for any damage that may occur during refinishing services of my firearm. If damaged items are found we will order parts to replace these items. I agree to pay for replacement parts if required. I agree to promptly pay Vitality Modification Company (VMC) in full for all charges of work performed on my firearm(s) and/or Item(s). I understand that my deposit money will be applied to my final bill. Please note excessively dirty equipment(fouled, loaded up, rust) will be charged an additional clean-up fee. We will call you to give you honest pricing.
